These forms are designed to support you in providing Doula service to your clients and to assist you with expanding your practice. By purchasing a Doula/Client Documentation Packet, you will have a system that allows you to maintain a professional and organized image. Your clients will appreciate your high degree of professionalism as you provide service that reflects your depth of knowledge. Your Doula/Client Documentation packet contains all the documentation necessary to support the entire client/provider relationship. The pages are not numbered, allowing you to put your documents in any order you choose. The left and top margins are adjusted for hole-punching. Included are:

Doula/Client Record - for recording vital information about your clients.
Appointment Notes - allows you to record appointments and keep notes during each visit. A space for driving directions, and map websites are included.
Phone Call Log - to record messages, action taken and advice given.
Financial Record - allows you to keep track of expenses for business and tax purposes.
Labor Record - for recording labor information as it occurs.
Doula/Client Agreement - a contract outlining in detail, the services you provide.
Doula/Client Questionnaire- a tool to guide you during home visits and to support and clarify the client/provider relationship. Includes resources for birth plan websites.
Support Check List - assists you in utilizing your skills as a doula, while providing your client with a list of comfort measures.
Doula’s Scope of Practice - Letter to give to hospital staff, clearly outlines your role and qualifications. A Compiled Doula Study is included to be copied on the reverse of the letter.

Pricing: Included in the $35 price of your Doula /Client Documentation Packet is the right to reproduce them for Individual Use. Organizations may purchase the Corporate Packet, $50, which gives you the right to reproduce the documents for your fellow colleagues. You will not need to re-order the packet, unless it becomes damaged or lost.
